Connolly Drive Medical Centre is conveniently located within the Lukin Centre, Captiva Approach, Butler, in a state-of-the-art medical facility. We are close to Butler Health Pharmacy and Butler Dental.
We have an on-site pathology collection centre (for blood tests etc.), clinical psychologist and Audiologist.
Appointments can be made by phone or by booking on-line via Website. Our standard appointment time is 10 minutes. If you require a longer appointment please let the receptionist know. Please note: we do not provide immunisations or wound care at weekend appointments as we do not have a nurse on duty.
We understand that your plans can change so if you are unable to attend a booked appointment please call reception. We do require two hours notice for all cancellations otherwise a fee is payable ($25 for all weekday & $50 for weekend /Public Holdiays). We Do not accept cancellations from 7am to 8am - if unattended a fee of $50 will be charged.
We strive to provide good clinical care and understand that some problems present urgently. We will always try to accommodate urgent problems on the day. We appreciate calling ahead to enable our team to triage your problem and provide safe and effective care.
Bulk billed home visits are available for patients who are unable to attend the practice or if the practice is closed. Please phone WADMS on 9321 9133. Home visits by our Doctors is at the Doctors discretion.
Please book an appointment with your doctor to discuss your test results. Our confidentiality policy prohibits our team from giving out test results by phone and email. For safe and effective care, it is important that patients consult the requesting doctor for test results.
Dr Antonietta Faulkner (clinical psychologist) is available for consultation at Connolly Drive Medical Centre. She accepts private clients with a GP referral and those with a mental health treatment plan.
Each doctor follows the principles of patient-centred care and aims to establish good professional relationships with patients. We understand that it can be frustrating when your usual doctor is unavailable but you are welcome to see other doctors in the practice.
Your medical record is a confidential document. Our staff adhere to strict confidentiality and privacy policies. Please contact the practice manager if your wish to discuss our policies.
Our doctors are unable to make referrals or issue prescriptions without a face-to-face appointment.
In line with our our privacy and data protection policies, patients may be contacted by phone or in writing about test results and other services. Please advise reception of your contact preferences.
Doctors are unable to accept phone calls whilst consulting. All calls will be triaged by a receptionist or nurse. If indicated, your Doctor will return your phone call. Emails will be received by reception and distributed to the necessary doctor. Email address: reception@cdmedical.com.au
Your medical record will be transferred to another practitioner or third party only upon receipt of your signed authority. A small fee will be charged for this.
Our facility has been designed with accessibility as a priority. A car parking space is reserved for disabled patients to the east of the surgery entrance. A wheelchair ramp is located adjacent to that bay. The surgery has an accessible toilet and all clinic rooms and corridors are accessible. Please call the practice manager if you have any questions about accessibility.
We recognise that things can go wrong and welcome feedback to help improve our services . We also appreciate positive feedback when things go well. You can provide confidential feedback using our suggestion box in the reception area.
